Check-ins & Updated Contact Information

Our staff may contact you periodically to check your treatment status or confirm your address, phone number, and other contact details.
If anything changes, please let us know so we can keep your case moving without delays.

2) Missed Work / Lost Wages

If your injuries caused you to miss work, ask your treating doctor for a work excuse note at each appointment
covering any days you missed.

If you are placed on restrictions or diagnosed with a disability, request a disability letter (if applicable).
You are responsible for requesting these documents from your provider.

Send your work notes and disability letter (if any) to our office along with your lost wage documentation.
If you did not miss work, you can skip this section.

3) Employer Wage Verification Form

If you missed work, your employer should complete and sign the wage/salary verification form confirming the dates you missed.
After your employer completes it, please mail it to our office.

Once we receive the form, we will attach your doctor’s work excuse notes for your claim file. If you did not miss work, you can skip this section.

4) Referrals (MRI, X‑Ray, Physical Therapy, Specialists)

If any provider gives you a referral—such as MRI, X‑ray, physical therapy, or a specialist—send a copy to our office.
If you did not receive any referrals, you can skip this section.

5) Prescription Costs (Out-of-Pocket)

If you paid out-of-pocket for prescriptions related to the accident, send us your receipts.

  • If you don’t have a receipt, you can send the prescription label from the pharmacy bag showing the medication information.
  • If you had no accident-related prescriptions, you can skip this section.

6) Medical Bills & Hospital/ER Mail

If you went to the hospital or ER, send us any bills or letters you receive from the hospital, emergency physician group, radiology,
or any other facility involved. If you have not received anything, you can skip this section for now.
